2025 NOTICE TO OBTAIN A COPY OF 2024 FORM 1095-C

This notice applies to all employees and dependents that were eligible to participate in Earl May Seed & Nursery’s Group Health Insurance plans during the calendar year 2024. Per IRS regulations made effective on December 23, 2024 through the “Paperwork Burden Reduction Act” (PBRA, H.R. 3797), employers are only required to distribute copies of ACA Form 1095-C UPON REQUEST.

To request a copy of your 2024 1095-C form, please contact us using one of the following methods:

1. Email your request to Marcy Nally, mnally@earlmay.com . In your email, please include: YOUR FULL NAME, PHYSICAL MAILING ADDRESS, EMAIL ADDRESS, and PHONE NUMBER. If you are a dependent you must also include the EMPLOYEE’s FULL NAME.
2. Request by phone by calling Marcy Nally at 712-481-3003 OR Angie Shaw at 712-481-3011.
3. Mail your request to:

   208 N. Elm St.
   Shenandoah, IA 51601

In your mailed request, please include: YOUR FULL NAME, PHYSICAL MAILING ADDRESS, EMAIL ADDRESS, and PHONE NUMBER. If you are a dependent you must also include the EMPLOYEE’s FULL NAME.

Your 1095-C form will be furnished to you within 30 days of receipt of your request. Any questions regarding your 1095-C form may also be directed to any of the communication methods provided above.
